Please choose one of the following registration options:
Information you need to have:
● What documentation is needed to register my child?
○ State issued birth certificate (long form)
○ Immunization record
○ Social Security Card
○ Medicaid Number and Copy of Medicaid Card
○ Parent Picture ID (driver’s license, passport, etc.)
○ Proof of income (all parents/guardians in home)
● Parents must also bring any two of the following:
○ Most recent utility bill with current address (water, electric, gas)
○ Most recent rent or mortgage statement with current address
○ Property tax statement with current address
